The personal information on this form is collected under the authority of the Municipal Elections Act, 1996 s. 27, 24 and 25 and the Assessment Act, 1990 s. 15 and 16 for purposes relating to municipal elections, voter eligibility and updating the assessment roll. Pursuant to the Municipal Elections Act, 1996, s. 88 this document is a public record, despite anything in the Municipal Freedom of Information and Protection of Privacy Act, 1990, and, until its destruction, may be inspected by any person at the City Clerk's Office at a time when the office is open.
